ANSWERS TO FREQUENTLY ASKED QUESTIONS:
Where do I go to take a SRI survey?
You will recive an email reminder for each class you are atteneding every week that the survey is open for. In these emails there will be a link to the survey. You can also go to the SRI channel in UVLink (see this screenshot) to take any open surveys.I have a class with Multiple instructors. Can I rate them all?
Yes. You should receive a survey link in an e-mail for each instructor assigned to your class. If you choose to submit the survey via UVLink, you will see link for each instructor in the "Student Rating of Instruction" box in the Attending Class page under the Student tab.
Can my teacher identify me or my results?
This survey protects your identity. The survey results are stored in a separate database and the only entry that identifies you in that system is encrypted. In order to insure that you only respond to the survey once per class, we must store a unique database ID for you. This ID has no identifying attributes, and is encrypted. No other personal information is stored in the SRI system. Your responses cannot be linked to you personally, or your student records. The survey results are given to the teacher in summary form, after grades are submitted.
I don't want to do it. Can I Opt out?
If you really don't want to or don't feel qualified to evaluate your instructors, you may opt out without any penalty. However, we strongly encourage you to take this opportunity to evaluate the quality of instruction at UVU. Your instructors use the survey results to improve the quality of teaching for future students. You opt out by clicking on the Submit Survey button without filling in the information. Once done, you will not be able to go back and respond to the survey later!
Can I preview the evaluation? What does it look like?

In addition, you can print a confirmation receipt from the Submitted Survey page. If you want to print this later, you need to keep your original e-mail with the survey link to access to confirmation receipt again.
How long do I have to complete it?
All surveys will be open for a total of 20 days, 14 before the end of the course and 6 days after. See schedule for exact dates for each part of term.

I'm not receiving any emails for me to evaluate my instructors with, what's wrong?
Make sure your email address is correct in Banner and that you are registered for the correct classes. If you need help in doing so, contact your advisor or
email email@example.com with an explanation of your problem.
Who do I contact for more information about evaluations?
Concerns and questions regarding evaluations should be sent to firstname.lastname@example.org. You may also contact the Faculty Center for Teaching Excellence or the Associate VP of Academic Affairs / Administration Kathren Brown .
If I can't take the survey or it doesn't work right, who do I contact regarding technical problems?
Please contact the IT help desk at (801)863-8888 for immediate response. You may also email email@example.com and include the course(s) that you can't evaluate along with your UV ID. Someone will respond within 1 working day.
