What do you need to get a UVU ID Card?
There are three things that you need in order to get a UVU OneCard/UVU PlusCard
1. You MUST be issued UVU ID Number
2. You MUST be registered for classes in the current semester
3. US Government-issued Photo ID
Valid
US Government-issued PHOTO ID
:
- Valid Driver's License
- State ID
- Passport
- Military ID
If government issued photo ID is not available, 3 forms of ID one of which must include:
- photo ID
- yearbook,
- parents driver's license (if student's birth certificate is present and the parent's name is on it)
- Non-resident Alien Registration Card
- Police Pistal Permit with Photo on it
- High School ID Card
- Club card with photo on it
- Credit card with photo on it
- Two other forms with name
- birth certificate,
- social security card
- parent's driver's license if name is on birth certificate,
- billing statements,
- bank statements,
- church records,
- Nonresident Alien Registration,
- Police Pistol Permit with Photo,
- Indian blood certificate,
- Court of Record documents,
- Item of mail with name and address typed
- Credit Card
If photo ID is not available, any of the 4 forms of ID will be required:
