Proposal Submission Form

If you are an undergraduate student enrolled in a college or university and would like to submit one or more presentation proposals in one or more areas (i.e. Best Practices, Research or Program Assessment), you must complete and submit a separate proposal for each desired presentation. While non-student staff, administrators are not invited to make presentation proposals they are encouraged to register for and (participate in the pre-conference 5K) attend the conference in support of their students.

All presentation proposals must be submitted no later than 5pm on Monday, April 1, 2013. Decisions will be made no later than Monday, April 8 and communicated prior to the April 9 priority (discounted) deadline.

If one (or more) of your presentation proposals is accepted we will waive your registration fee to the conference. However, if you are interested in eating the lunch provided at the conference, the cost will be $10.

READ CAREFULLY:
If your proposal is approved and you do NOT want to eat the conference lunch, to register please email ce@uvu.edu with your first and last name, email, phone and college/university name. Please also attach the presentation approval email (we send you) as proof of your presentation approval.

If your proposal is approved and you DO want to eat the conference lunch, please call 801-863-8012 to register for the conference.

For each desired presentation proposal, complete and submit the form below. Questions about presentation proposals can be directed to slamconference@uvu.edu. You may submit more than one presentation proposal.
