To change the address for your Perkins Loan, please send the following information to the UVU Collections Office. You may call, write, email, fax, or stop by the Collections Office.
  1. Name (previous name if submitting a name change)
  2. Student ID Number
  3. New name (if submitting a name change)
  4. New address
  5. Home telephone number (including area code)
  6. Work telephone number (including area code)
