A cancellation fee of $100.00 will be charged if CRFS is not notified within 2 weeks of scheduled visit.
A scheduling change fee of $25.00 will be charged if changes occur within 4 weeks of the prior scheduled visit.
All fees, including deposits, are subject to change and must be paid in advance.
