DEPARTMENT ADDENDUM FOR ALL MAT 0800 CLASSES

Course Fee:
The fees for this class, which you have already paid with your tuition, will cover the costs of the syllabus, department policy handout, consumable tests, scantron forms, project handouts, manipulatives, study aids, upgrade of current tutorials and videos available in the math lab and in the library, and replacement of lost or stolen videos and software.

Attention Students With Disabilities:

Academic Accommodations are granted for all students who have qualified documented disabilities. Services are coordinated with the student and instructor by the Accessibility Services Department.

Learning Assistance:
The University College offers learning assistance to students who are having problems with test taking, concentration, attendance, and all types of study skills. You may contact Pat Nelson, the learning strategist, in LA 221p or call her at 863-7418. If you have any diagnosed learning or physical disability which may impair your ability to successfully complete this course, please let the instructor know at the first class meeting.

Math Lab:
The Math Lab, located in LA 201, is a free service to you. Tutors are available to give you one-on-one help with concepts, skills and applications. Software to aid you in your coursework may also be available. The Math Lab hours are Monday through Thursday 8 a.m. to 9 p.m. The math lab will also be open on Friday from 8 a.m. - 5 p.m. for group study sessions. (All hours, as well as possible Saturday hours, are pending funding.)

Academic Honesty:
No academic dishonesty will be tolerated in this course. The penalty for a first offense is an "F" for the assignment; the assignment cannot be redone. A second offense means that you fail the course and will be reported to the Department Chair and to Student Advising. Below is the definition of cheating taken from the current Utah Valley University Catalog. This definition does not apply to group work assigned by the instructor that you have actively participated in. Please read the complete "Student Rights and Responsibilities" section in the current UVU catalog so that you are aware of your academic responsibilities.

Cheating:
Cheating is the act of using, attempting to use, or providing others with unauthorized information, materials, or study aids in academic work. Cheating includes, but is not limited to, passing examination answers to or taking examinations for someone else or preparing or copying others' academic work

Dropping the course/Incompletes:
Students who choose to drop the course must complete the drop process by the date listed in the course catalog. Failure to drop the course by the drop deadline listed in the course catalog will result in a UW if you were passing the class on your last day of attendance or an E if you were failing the class on the last day of attendance. To qualify for an incomplete you must have completed 80% of the coursework with a passing score and have a medical or employment emergency that physically prevents you from attending class. The department chair will require documentation.

Midterm/Final:
A departmental final exam will be given in this course to assess outcomes. Each instructor will decide whether or not to give a midterm based on the instructor's educational philosophy. A minimum of 25% of your overall grade must be assigned to a midterm and/or a final, with a minimum of 20% from the final.

Calculators:
Due to the nature of the material covered and the course objectives, no calculators are allowed in this course.
