Because of federal mandates, each school year has different requirements that must be satisfied. Please select the correct school year and form from the list below. Forms completed for an incorrect year can not be accepted and may delay your receipt of financial aid.
Except for the FAFSA and UVU Data Form, which all students complete, students will be informed through their UVLink if they need to complete any additional forms. When turning in hard copies of any documents, be sure to write your UVID number on each page. You are encouraged to make duplicates of all documents submitted for your own personal record. Check your status frequently and submit requested forms timely to complete your financial aid file. These forms are made available here for your convenience.
