If you want to work on campus, the department hiring you must submit this form to the International Center WB-100. |

In order to receive a Social Security Number and Card, you must already be hired. |

Request for New I-20 |
If you lost your I-20, need to renew your I-20, have changed your major, or are changing your academic program, please submit this form to an International Adviser. |
| Request for Letter of Certification | If you need a letter of certification for financial or enrollment purposes, please submit this form to an International Student Adviser. |
| Transfer Out Form | If you will be transferring to another school, please submit this form to an International Student Adviser. |
| Departure Notice |
If you will be leaving UVU, this form must be completed and turned into the International Adviser. |
