- Review the Team Packet (pdf) & Download the Team Captains Declaration Page (pdf), and fill out all paper work.
- Download extra Risks and Hazards Waiver for each team member.
- Review the 5 on 5 Basketball Info Sheet (pdf) ... & turn in ALL completed paperwork to SC-108h by Friday Jan. 8th @ 4 PM.
Then attend the
"Mandatory"
Captain's Meeting on Monday Jan. 11th at 4 or 5 PM in SC-105...
Deadline for additions to rosters is Friday January 22nd @ 4 PM ...
