Prior to acceptance into the MBA program

The following items will be required in order to issue an I-20

  1. Affidavit of Support. Form I-134 from U.S. Department of Homeland Security. May be accessed by clicking here. This form is to be completed by the individual sponsoring a student planning to attend Utah Valley University.

A student may request UVU to issue them an I-20 Certificate of Eligibility in order for them to obtain a student visa. Before UVU is able to issue the I-20, the student must provide proof of financial responsibility. With this statement of support, the sponsor assumes the financial responsibility.

In addition to this form, sponsors must also send an original financial statement to the MBA office showing sufficient funds to cover the student schooling and other expenses. All financial support papers must be in English, showing amounts in U.S. dollars.

Note: The U.S. Department of Homeland Security now requires the completion of Form I-901 and a fee remittance of $200.00 prior to your appointment for a Visa. The form and instructions will be included at the time the student is issued an I-20.
