Spoiled Check Procedure

Forfeiture of Privileges
Persistent improper use of the Limited Purchase Check program will result in the withdrawal of the privilege to use this means of making small dollar purchases.
The following actions will be taken if violations occur:
- 1st violation will result in a notification being sent by Purchasing regarding the improper use of LPCs.
- 2nd violation will result in three (3) month probation. All unused checks must be returned immediately. If checks are not returned, the privilege to use the LPC program may be withdrawn.
- 3rd violation will result in twelve (12) month probation. All unused checks must be returned immediately. If checks are not returned, the privilege to use the LPC program may be withdrawn.
- 4th violation will result in withdrawal of the privilege to use this program. All unused checks must be returned immediately.
If after twelve (12) months no further violations occur, then the previous violations will be purged. Purchasing will review each used check to determine the appropriateness of the expenditure and to verify that proper policy and procedures were followed. If after review by Purchasing, improper use is determined, a violation will be placed on the expended Index. Check custodians may receive one-on-one training at the discretion of purchasing. After the training session, Purchasing may remove violations.
