Please call to make an appointment at 801.863.8876. If you are unable to make your appointment, please
give 24 hours notice. A $5.00 fee will be charged for all no show appointments.
Mental Health Services
Hours of operation:Monday through Thursday 8 am to 8 pm, Friday 8 am to 5 pm; closed weekends and holidays.
Please call 801.863.8876 and inquire about making an appointment to see a mental health professional. If you would like to speed up the process when making your first visit please print and fill out the forms below and bring them with you to your first visit.
