CTC Course Request Process and Online Instructions

Course Request Process

Prior to requesting a course please complete/understand the following:

  1. Print the Course Request Form.
  2. Have an enrollment roster of 12 students, which contains an email address for each student. The enrollment roster may consist of students from multiple agencies.
  3. Requested course dates must be provided and be a minimum of 60 days from the date of course request.
  4. CTC courses are completed through blended learning, which includes internet based course work and testing followed by 16 hours of classroom work. The classroom work is completed on 2 consecutive dates in the CTC Facility but may be divided into consecutive four and/or eight hour blocks.
  5. The requesting agency's Point of Contact contacts UFRA at 801-863-7700 (Utah County), or toll free at 888-548-7816, and speak with a program manager for your area.

After the course request has been placed the following will occur:

  1. The Program Manager will conduct an internal confirmation of course availability, at which time the Program Manager will confirm course dates with the Point of Contact.
  2. The CTC Administrator will contact the requesting agency's Point of Contact with online account information for each student. It is the responsibility of the Point of Contact to circulate the online account information to each student.
  3. 30 days prior to the course, the CTC Administrator will conduct a progress status check of the online course work for each student. A minimum of 10 students must have completed the online course work with successful completion of each quiz and the CTC entry exam. The CTC Administrator will notify the Point of Contact with the results of the progress status check. If a minimum of 10 students have not met this requirement, the course will be cancelled. In the event that the course is cancelled, the Point of Contact must select new course dates that are no sooner than 45 days from the previously scheduled course.
  4. If applicable, 15 days prior to the course, the CTC Administrator will conduct a final progress status check of any remaining students. The CTC Administrator will notify the Point of Contact with the results of the final progress status check.
  5. The final course roster will be created 15 days prior to the course. There will be no student substitutions allowed after the final course roster has been created.
