If you are on the waiting list and have been notified that we have room for your child/children please complete an application (pdf). Make sure it is filled out correctly and all papers are signed. Please bring the application to the center.

If you have a child that will be under one year make sure that you fill out an About Your Infant (pdf) form.

If you have more than one child please fill out the following for each additional child:
