Basic INFORMATION

The U.S. Department of Education has a process to verify financial information in order to ensure accurate data is being reported on the FAFSA. If you are selected for verification, it is required that you complete the process in order to receive your federal student aid.

To view your required tasks to complete your financial aid file, create an account and log in to StudentForms at https://uvu.verifymyfafsa.com/account/Login.



Blue 2COMPLETE AND UPLOAD ALL REQUIRED VERIFICATION DOCUMENTS

StudentForms will guide you in completing your tasks. There are web forms that must be completed within the StudentForms portal, and other documents are uploaded by using the Upload button listed under each specific task. Conveniently complete and upload documents from your computer, and other mobile devices.

Household forms are signed electronically using your password.


Blue 3BEGINNING WITH THE 2018-2019 AID YEAR, USE THE IRS DATA RETRIEVAL TOOL

The IRS Data Retrieval Tool (DRT) is not available for the 2017-2018 aid year. This affects students who wish to transfer their tax information directly from the IRS to their FAFSA.  If you are required to provide tax information to the Financial Aid and Scholarships Office for verification, and did not previously use the DRT, you must provide an IRS Tax Return Transcript.

How to use the IRS Data Retrieval Tool:

          • Go to www.fafsa.ed.gov
          • Go to the Returning User? section and click on Login. Do not submit a new FAFSA, make corrections to your current FAFSA.  
          • Enter your FSA ID or Student Information and click Next. 
          • If requested, enter your Save Key. 
          • Click on the appropriate year tab. 
          • Click on Make a Correction.
          • Click on the Financial Information Tab. Under the Student and/or Parent Financial Information, answer the questions in the “To determine if you can use the IRS Data Retrieval Tool to transfer your tax return information from the IRS into your FAFSA, answer the following question(s):” box. 
          • If eligible click on “Link to IRS” box. This will show a “Leaving FAFSA on the Web” page. Click OK to continue.
          • You will receive a popup box from the U.S. Government, read the information and click OK.
          • Enter your street address, city, state, and zip code exactly as it appears on your 2015 Federal Income Tax Return and click Submit. 
          • Your tax information will appear. Mark the box “Transfer My Tax Information into the FAFSA.”
          • Click the “Transfer Now” button.
          • Do not update the information on the questions that have “Transferred from the IRS”.
          • Enter your wages from work (if married enter your wages and then your spouse’s wages, if your spouse earned wages in 2015.  For your parents, enter your father’s earned wages and then your mother’s earned wages).
          • Answer the dislocated worker question.
          • Click NEXT remember, do not update the questions that have “Transferred from the IRS” notated.
          • Sign and Submit your FAFSA again. 

If you are unable to use the IRS Data Retrieval Tool, you have one of the following options:

          • You may request your IRS Tax Return Transcript online at www.irs.gov to have your IRS Tax Return Transcript mailed to the address listed on your taxes (see steps below).
          • You may contact the IRS at 1-800-829-1040 to request your IRS Tax Return Transcript. Tell the employee that you need an IRS Tax Return Transcript for educational purposes.
          • You may use the IRS Form 4506TEZ, which is a request to obtain your IRS Tax Return Transcript. You can obtain the IRS Form 4506TEZ at www.irs.gov/pub/irs-pdf/f4506tez.pdf. Once you complete the form, you may mail or fax the form to the IRS at the address or fax number they provide on the form.

How to request your IRS Tax Return Transcript online (to be mailed to address listed on your taxes):

          • Click on “Get a Tax Transcript” under the tools menu
          • Select Get Transcript by MAIL
          • Read the information on the pop-up box and click “OK”
          • Enter all personal information for the primary taxpayer and “Continue”
          • Choose the Type of Transcript (Return Transcript)
          • Select the Tax Year
          • Click “Continue”
          • You will receive a confirmation screen that says: “We have accepted your request for a 2015 Return Transcript. Please allow five to ten days to receive it. The transcript will be sent to the address we have on file for you.”
          • If you were not required to file an IRS tax return, you must provide a Verification of Non-filing Letter from the IRS.
          • Allow two weeks for processing.
          • If additional information is necessary, we will contact you through phone, your myUVU email, or notify you under your Financial Aid Requirements in your myUVU student account.

Verification DEADLINES

For the Pell Grant Program: Submit completed verification documents no later than 100 days after the last date of enrollment.

For the Direct Loan and Campus-based Programs: Submit completed verification documents no later than two weeks before the last date of enrollment.

PROCEDURE

The Financial Aid and Scholarships Office will use the documentation you provide to verify the information you reported on the FASFA.  If it is determined that there are discrepancies, corrections will be sent to the U.S. Department of Education. The U.S. Department of Education will review the information and may request further documentation.  If additional information is necessary, we will contact you through phone, your myUVU email, or notify you through StudentForms.

If previously awarded federal student aid: If you were previously awarded federal student aid and your Expected Family Contribution (EFC) changes, your federal student aid award may be adjusted. You will be notified through your myUVU email account if a change in your award occurs. You can view your awards in your myUVU student account.

To view and accept your award, log in to myUVU, click on Students, Paying For My Education, My Financial Aid Awards, Select the appropriate Aid Year, Terms and Conditions, Accept Award Offer.
