Vehicle Acquisition Request Form

UVU Departments seeking to acquire a vehicle must submit a Vehicle Acquisition Request form signed by the Dean or Director, Fleet Manager and Vice President of Administration & Legislative Affairs. This would include new vehicles, replaced vehicles, surplus and donation vehicles. Donation vehicles need to go through the Foundation Department for approval.
