Please note that there is a charge for a proximity access device, and your department
must provide their index for this charge. If you already have an access device (smartphone or fob), there is no charge for additional access.
When using mobile access, you will receive an email from HID Global with instructions
to download the app. DO NOT install the app until you receive the official email from
HID Global.
Use this form to request access to interior campus doors using a proximity access
device. This form should be filled out, completely, by a UVU Administrative Support
staff member or by the employee requesting access.
- Select the link below.
- You will be directed to a digital form, fill in the requested information and click
the “Finish” button.
- An email will then be sent to the person being granted the requested access. This
person should review the form, sign electronically as indicated and click the “Finish”
button.
- An email will then be sent to the supervisor. The supervisor should fill in the requested
information, sign electronically and click the “Finish” button.
- The form will now be directed to the UVU key shop for further action. After this process
the person who is being granted access will need to follow one of the following:
- If the person is being granted access on their mobile device, they will receive an
email from HID Global with detailed instructions on downloading and using the mobile
application.
- If the person is being granted access on a separate key fob, they will receive an
email from Facilities stating that the device is ready for pickup.
