To report an accident, near miss / near hit. property damage or other incident please submit the relevant accident form on the Risk Management website.

Request Safety Audit and Inspection

The Safety office performs audits and inspections to identify areas of potential improvement.

If you have a concern about a possible safety related issue request an audit or inspection.

Examples of audits that may be performed:

• General safety
• New construction/new task safety evaluations
• General chemical safety (chemical storage, use, and compatibility) 
• Industrial hygiene, including:
  • Ventilation (general ventilation or local exhaust)
  • Mold
  • Chemical exposure
  • Noise
  • etc.
• Electrical
• Confined space
• Personal protective equipment evaluations and determinations
• Other safety audits and inspections as requested.

Hazards and Suggestions

Have you identified a potential safety hazard? Report it here. 

Please include the following information:

• Location 
• Specific hazard or suggestion
• Name and contact info (so we can follow up and reply back)
• Please include suggestions or possible actions to correct the hazard.

Training

Creating and maintaining a safe environment for employees, students, and guest is often closely related to an individuals understanding of the hazards and controls for those hazards. The EH&S Coordinator strives to create training opportunities to develop an understanding of relevant hazards and how to mitigate those hazards while on UVU campus.
