UVU Employee or Student/Visitor Incident Reporting
- This is a single report form to report INJURIES, SAFETY CONCERNS, AUTO ACCIDENTS, and INSURANCE CLAIMS against UVU. This report form is intended for safety and insurance matters, including
WORKERS' COMPENSATION.
- Employees should use this form to report workplaces injuries or illnesses. However,
employees should first report incidents first to their supervisor and/or People and
Culture. Then, they should seek appropriate medical care given the situation. Lastly,
they should complete this form.
- Students and visitors should use this form to report incidents such as general liability
claims against UVU.
- In general, UVU's general liability insurance does NOT provide coverage to students
in the classroom. However, all claims are determined on a case-by-case basis by Utah
State Risk Management. Claims must be formally submitted to the responsible department
(People and Culture, Risk Management, etc.) such as via this form, for coverage considerations.

This form will automatically send a copy to Risk Management, People and Culture, and
Safety.
At the end of the form, you can add another email to be notified (such as a supervisor).
