ALL applicants must be admitted to Utah Valley University before applying to the Dental Hygiene Program.

Applications for admission to the Dental Hygiene Program are only accepted during the open submission period.

Applying

Application and all application materials (sample application, transcripts, HSRT scores, Clinical Observation Verification, and reference contact details) must be submitted by 4 pm on February 1. Only completed applications will be considered for admission. To be complete, all transcripts and test scores must be official and in sealed envelopes.

It is the applicant’s responsibility to make sure their application is complete and has been received before the deadline. Applicants may call the academic advisor to check if certain application materials have been received. Advanced planning is strongly encouraged. Deadlines will not be extended for individual applicants. 

 

Required Materials

• Health Science Reasoning Test Results as jpg or PDF
• Dental Office Observation Form with appropriate signature as jpg or PDF
• Name and email for 2 references. We will request the reference from email provided.
• 2 page, double spaced, essay about yourself and why you are a good candidate for the UVU Dental Hygiene Program. 

All application materials will be submitted online as part of the application process. All communication will be through the applicant’s UVU student email.

 

Are you reapplying for the Program?

If you are Reapplying for the Dental Hygiene Program at Utah Valley University you will be required to submit a NEW application through the application portal. We are unable to reuse any documents from any previous applications.

Each applicant must submit ALL required documents for each admission cycle.

Applicants are reviewed by number only, not by name, for computation of points. The top 20 applicants will be admitted into the program. Upon acceptance into the program, applicants will be required to complete and submit drug testing and background check to hold their spot in the program.
